Order Northwest University Transcript for Concurrent Credit
The best way to order Northwest University transcripts is through www.nationalstudentclearinghouse.org (NCS). Students will input some identifying information, payment method, and where to send transcripts. After you have submitted this information, NU will receive and process your request.

Note: There is often an error message that pops up for concurrent credit students when ordering online. The error comes through because NSC may not have your NU Student ID or Social Security Number (SSN) on file, so the system does not recognize you. The best way to move past that is to select “No” when it asks if you would like to edit your personal information above. Then you should be able to go forward and continue entering information to complete your order. We will then verify your information at the school. If you don’t have an SSN, you can use “000000” as your student ID number and follow the same instructions.

Upper School Forms

Submit this form to the Upper School Division Head after meeting with each teacher.
Sports Physical Evaluation and Medical Eligibility Form
Required annually for students in grades 6 – 12 who participate on Bear Creek athletics teams.

The Sports Medical Eligibility Form (page 1) is valid only if it has been signed and dated by a physician and uploaded to the health portal. Do not upload the Sports Physical Exam Form (page 2).

International student-athletes may receive the sports physical exam in their home countries with their own doctors; however, the Sports Medical Eligibility Form must be completed in English.

Forms Requiring a Health Care Provider Signature

These forms must be updated annually by your child's WA licensed health care provider and uploaded to Bear Creek's health portal as part of Back-to-School Registration. Do not mail, email, or drop off at school. Thank you!

Allergy Action Plan
Required for any student with a life-threatening allergy who is prescribed an epinephrine autoinjector (e.g., EpiPen, Auvi-Q, etc.).
Asthma Action Plan
Required for any student diagnosed with asthma or who is currently prescribed an inhaler.
Seizure Action Plan
Required for any student diagnosed with a seizure disorder.
Authorization to Administer Medication
This form is:
- Required for any student prescribed emergency medications (e.g., epinephrine, inhalers, insulin, or seizure medications).
- Required for any student who needs to take medication during the school day.
- Required for any medication that students in grades 7 – 12 will self-carry/self-administer during the school day or on an overnight trip.
Authorization to Administer School Provided Over-the-Counter Medication
Use this form to indicate which over-the-counter medications the school may provide to your child.

Immunization Forms

Certificate of Immunization Status

This immunization document must be uploaded to the Bear Creek health portal. Do not mail, email, or drop off at school. Thank you!

Please use one of these options to retrieve your student’s official Certificate of Immunization Status document:

Sign up for https://myirmobile.com/ where you can directly access your student’s document.
Request a complete immunization record from your health care provider, clinic, or your local pharmacy.
Request a complete immunization record from the Director of Health Services Lela Powell at epowell@tbcs.org. Requests must be made before August 1.

The Bear Creek School uses the Washington State Immunization Information System’s School Module for efficiency in insuring that each student has the documents necessary per legal requirements.

Students who are coming from out of state or country must upload a completed CIS form that is signed and dated. If the CIS form is hand-filled and signed by a provider or stamped by a clinic, no secondary proof of immunization is required. If the CIS has no provider or clinic validation, additional proof of immunization must be uploaded in conjunction with the completed CIS.

Valid proof of immunizations as they have been filled out on the CIS document incudes:

Records from a clinic, provider, hospital, or registry with an official logo or stamp
Official foreign or out-of-state immunization records that include provider validation

Certificate of Exemption Document

Students who do not meet the state immunization requirements must have an uploaded Certificate of Exemption that has been signed by both the parent and the student’s health care provider.

Parent Forms

Background Check Disclosure
If you wish to volunteer or work with children of any age in or out of the classroom, print and complete this form, then submit online by uploading a photo of the form with your valid driver's license to the health portal. Volunteers must submit this form annually. This form must be submitted in the health portal as part of Back-to-School Registration.
Volunteer Driver Acknowledgement
If you wish to drive on field trips or to class parties/events this year, print, complete, sign, and submit online by uploading a photo of this form to the health portal along with proof of auto insurance. Volunteers must also submit the Volunteer Background Check Disclosure. This form must be submitted in the health portal as part of Back-to-School Registration.
