The best way to order Northwest University transcripts is through www.nationalstudentclearinghouse.org (NCS). Students will input some identifying information, payment method, and where to send transcripts. After you have submitted this information, NU will receive and process your request.
Note: There is often an error message that pops up for concurrent credit students when ordering online. The error comes through because NSC may not have your NU Student ID or Social Security Number (SSN) on file, so the system does not recognize you. The best way to move past that is to select “No” when it asks if you would like to edit your personal information above. Then you should be able to go forward and continue entering information to complete your order. We will then verify your information at the school. If you don’t have an SSN, you can use “000000” as your student ID number and follow the same instructions.

Required annually for students in grades 6 – 12 who participate on Bear Creek athletics teams.
This form is valid only if it has been signed and dated by a physician. Submit page 4 online by uploading to health portal. Do not upload pages 1 – 3.
International student-athletes may receive the sports physical exam in their home countries with their own doctors; however, the Physical Examination and Medical Eligibility Forms must be completed in English.

Required for any student with a life-threatening allergy who is prescribed an epinephrine autoinjector (e.g., EpiPen, Auvi-Q, etc.).
Required for any student diagnosed with asthma or who is currently prescribed an inhaler.
Required for any student diagnosed with a seizure disorder.
Required for any student prescribed emergency medications (e.g., epinephrine, inhalers, insulin, or seizure medications).
Required for any student who needs to take medication during the school day that is not included on the list of nine parent-approved over-the-counter medications available in the health room (e.g., ADHD medications, antibiotics, etc.).
Also required for any other medication that students in grades 7 – 12 will self-administer on an overnight field trip.

The school may only accept the medically verified Certificate of Immunization Status which has been uploaded to the health portal. We recommend printing this form directly from wa.MyIR.net or asking your health care provider to print directly from the Washington State Immunization Information System; however, you may complete the form yourself and have your health care provider sign it or submit it with medical records attached. Required for all new students (includes all P3) plus returning preschool, kindergarten, and grade 7 students.
In 2019, the Washington State Legislature passed a bill that removes the personal and philosophical option to exempt children from the MMR (measles, mumps, and rubella) vaccine required for school and child care entry. This form implements the law.

Volunteers must submit this form annually. This form must be submitted in the health portal as part of Back-to-School Registration.
