International student-athletes may receive the sports physical exam in their home countries with their own doctors; however, the Physical Examination and Medical Eligibility Forms must be completed in English.
Parents of students entering grades 5 – 8 may complete this form to provide additional information to the class placement process.

The school may only accept the medically verified immunization records which have been uploaded to the health portal. We recommend printing this form directly from wa.MyIR.net or asking your health care provider to print directly from the Washington State Immunization Information System; however, you may complete the form yourself and have your health care provider sign it or submit this form with medical records attached. Required for all new students (includes all P3) plus all kindergarten students and all grade 7 students.
In 2019, the Washington State Legislature passed a bill that removes the personal and philosophical option to exempt children from the MMR (measles, mumps, and rubella) vaccine required for school and child care entry. This form implements the law.
